Enrollment Process
The Center recognizes the unique characteristics of each person served and uses a holistic approach in providing services specific to need. This approach requires input from the client to the best of their ability, their families, their guardians, caregivers, advocates, professional and direct care staff.
Prospective clients, their families, guardians or caregivers contact the center to schedule an appointment to visit the Lucy Idol Center. It is important for the prospective client to also attend this visit to see the facility and to meet the staff. During the visit they will learn about the services provided, staffing, environment, rules, safety, transportation and will provide information about themselves and specified needs.
- Tour the building & grounds and meet the staff.
- Meet with the Program Director and the Registered Nurse to complete the Pre-Admission Information Sheet.
- Review of the yearly calendar and the Client Handbook that includes Client Rights and Due Process.
- They will receive information about services and fees as appropriate.
- The Lucy Idol Center’s policy for a 90 day probationary period will be reviewed. It states that each individual enrolled will be have probationary status for 90 days to determine if services provided by the Lucy Idol Center are appropriate to need.
Following the center visit, the prospective enrollee and others involved express their desires for enrollment. The Program Director and other administrators including the Executive Director review provided information to make a decision about enrollment.
The decision may be made during the visit or soon after. If a decision is made for enrollment the prospective client makes an appointment for an eligibility/enrollment/evaluation plan meeting. If it is decided that services will not be provided by the Lucy Idol Center – The Program Director will make referrals to outside agencies or support systems.
- A meeting will be scheduled and the client, their family, caregivers, guardian and advocates will attend to proceed with the steps for the intake process.
- An intake form will be completed that includes personal information, evaluations, eligibility determinations, specified enrollment services, special considerations and accommodations, indications of functioning, co-occurring disabilities, effective re-enforcers, assessments/evaluations to be completed, team attendance, approval or referrals if applicable.
- This meeting will include a comprehensive orientation and persons involved will be provided with the necessary intake forms.
- Daily Transportation will be discussed for scheduling.
- The initial Individual Service Plan meeting will be scheduled as part of the enrollment process.
