Program Description: Day Habilitation: Adjunctive Therapy & Diagnostic Assessment
Purpose
The Lucy Idol Center program provides program services to mentally, physically, and developmentally handicapped individuals with training and related services which enable them to increase their control and independence in the environment. It provides recreational therapy in the community setting and a balanced program development emphasis to improve and develop functional skills and overall improved quality of life.

- Physical disabilities related to neurological handicaps and related disaffection.
- Functioning emotionally disturbed individuals
Criteria for Consumer Admission
- Resident of Lorain County or Erie County.
- Adults accepted
- Diagnosis as described above in target population.
- Physical and social adaptability to the program.
- Referrals from physicians, other agencies, or community.
Intended Client Objectives and Priority Services
Service outcomes are ongoing and describe the individual's movement or status change as a result of Intervention Services. Daily documentation helps determine progress or regression. 180 day reviews determine continuation, change, or discontinuity of specific goals. Performance Improvement helps to determine program and service evaluation and appropriateness. Length of stay is dependent upon each individual's personal need and situation.
Sample Objectives:
- Enhance community integration/developing and maintaining social relationships.
- Development of functional skills to lessen dependence and enhance potential for accessing social supports necessary to promote the successful functioning of the individual in the community.
- Health maintenance and physical development.
Screening
Intake: Initial contact with prospective client and caregivers. Eligibility. enrollment, and evaluation evolves through an interdisciplinary team process which determines the need for further services, testing, - assessment.

- Medical Information
- Psychological
- Previous Agency Records or consultations
- Social History Summary
- Pertinent correspondence
- Medicaid card (if eligible)
- Date set for ISP (30 days from intake)
Assistance To Speakers of Other Languages and the Hearing Disabled:
Assistance will be provided, as appropriate according to the person's needs, at no additional cost to persons served, to persons requesting or receiving services, and their families or significant others, who speak a language other than standard English as a primary means of communication, or who have a communication disorder, such as deafness or hearing impairment. The Center has a telecommunication device for the deaf ( "TDD') to assist individuals who are deaf.
Client Involvement:
The needs of people with multiple disabilities can be complex. Meeting the needs demands the knowledge and expertise of many people. The Center's evaluations for client input are maintained through the interdisciplinary team process. This is the most effective mechanism for insuring that the combined expertise of all involved is best utilized to meet the needs of the individual served. The sharing of knowledge and expertise and the integration of that information by consumers and providers of service creates a plan that is comprehensive and coordinated for organizations, individuals, and referrals for other service providers.
Cultural Competency:
We assure equity of program services to all individuals regardless of race, sex, age, religion, national origin, or handicapping condition. This includes participation by individuals from all cultures and handicaps in decision making, program design, and evaluation. The Center maintains compliance with the Affirmative Action Plan and Section 504 of the Rehabilitation Act of 1973.
Systems Coordination:
The Individual Service Plan (ISP) refers to the written plan of intervention that is developed by the interdisciplinary team. It specifies both the goals and objectives being pursued on behalf of the individual and the steps being taken to achieve them. It identifies skill development that outlines progressive steps and the anticipated outcomes of services. Individual service planning is a continuous, coordinated and self-correcting process.
